There are plenty of different things that you can change and customize when you design a horse in Create-A-Sim, such as their breed, size, facial structure, traits, and more. Much like the pet Create-A-Sim functions for Cats & Dogs, you’ll be able to design the horse of your choosing in CAS.

Age And Traits
Before you design what your horse looks like, you’ll need to add your horse to the family and figure out its name, age, and traits.
Much like human (or supernatural) Sims, you’ll be able to give your horse a name and select how old they are: whether they’re a baby foal, a full-grown adult, or an elder horse in their golden years.
Additionally, making your horse in CAS in The Sims 4 allows you to choose between 11 unique traits for them.
This gives you a huge advantage over horses you’ve adopted, since you can’t choose a horse’s traits anywhere outside of CAS.
How To Change Your Horse’s Appearance
Once the basics are decided, you can begin to customize nearly everything about your horse, from various facial features to tails and manes, and you can even give them a wide selection of fun accessories, too.
You can change their manes, tails, patterns, saddles, horseshoes and so much more by clicking your horse’s body in CAS to bring up the menu of premade items and patterns.
But, if you’re not interested in tweaking any of the sliders or choosing extra gear, you could always choose a breed to get started on creating a custom horse.
Though it doesn’t make a difference for their traits, nor does it lock your horse into any strict appearance after further customizations in CAS, you can choose from more than 30 unique horse breeds to get started on making a horse of that breed.
Different breeds, by default, have different appearances in terms of things like manes and tails, muscle definition, height and width, markings, and more.
You can, of course, change the majority of this as well, but different horse breeds are a great way to get a head start when designing a horse in The Sims 4.
The Paint Tool
Finally, if you’re feeling a bit creative, you could always opt to paint designs on your horse yourself by hand using the Paint option.
You can click the upside-down horse icon at the top of the screen to access its belly during painting.
Here, whether it’s an artistic design for your horse’s coat, a couple of birthmarks here and there, or whatever fun ideas you come up with, you’re free to design your horse’s coat as you please in this mode.
